Clutch pedal rod bushing

The original bushing finally broke a few months back. I bought a replacement but it did not last. Just bought another one and it lasted a couple days before the rod came off again. Two of the tabs are broken now. Is there something that goes along with the white bushing that keeps the rod from coming off the pin on the pedal? Just wondering why I’m having an issue now.

I took a good look at the clutch pedal in my truck today. I noticed that firewall was cracked in a few places. After welding the cracks up it was more stable but the rod and bushing still wants to try and come off the pin. Drove the truck a few miles and i could tell it moved and the pedal changed. Is there a retainer that I am missing?
